A med spa can look busy and be stuck. The injectors are full, the front desk is answering the phone, somebody is posting to Instagram, and revenue is doing roughly what it did last year. Boulevard's industry data puts the typical med spa at a 20 to 25% margin, which doesn't leave room for marketing that produces activity instead of patients.
The block in a clinic is usually one of four things. The patient searched "Botox near me" and found the clinic two plazas over, because your location pages don't exist or don't rank. She called and the front desk informed her instead of booking her. She came in once and nobody brought her back on the 12-week cycle, so she drifted to whoever ran a special. Or the clinic is still being measured by the channel that built it, and nobody can see the patients who never called. That last one was Skin Vitality, whose print still worked.

It starts with an audit of the clinic: each location's search presence, where bookings come from and what they cost, what happens on the phone (we listen), how often a first-visit patient comes back, how the specials are affecting the fee. For a multi-location clinic we do it per location, because the patient in Mississauga doesn't want Richmond Hill.

Prescribe
A short plan with the fixes in order and who owns each one. Often that's phone training and a rebooking sequence before any new ad spend, then location pages and local search, treatment-level Google Ads, a site built to book, and call tracking on every channel so the cost of a booked appointment is a number instead of a feeling.

For media, most clinics should plan on $6,000 to $30,000 a month; non-surgical and surgical practices land at different points in that range. There's no long-term contract. The first engagement runs three months, and clinics stay after that because bookings are up, not because they signed something.

How is this different from a med spa marketing agency?
An agency sells you a channel. We start by finding out what's actually blocking the clinic, and the answer decides what gets built. Skin Vitality didn't need more ads. Their print still worked. They needed to exist where patients had started looking, one local search result at a time, and then they needed each new clinic to open into a market we'd already won. That's a different job from running campaigns.
What does it cost?

How long until it produces bookings?
Phone fixes pay off in the first month, because they recover patients you're already paying to reach. Google Ads produces booked appointments within weeks when the front desk can handle them. Local search takes four to six months to move and compounds from there, and for a multi-location clinic every location is its own race. Retention work on the 12-week neurotoxin cycle starts paying the first time a patient who would have drifted gets a reminder.
Is there a long-term contract?
No. The first engagement runs three months, which is long enough to give the process a fair chance to start working. After that, clients stay because the numbers are moving, not because a contract says they have to. Media is separate and sits in your own accounts. Most clinics should budget $6,000 to $30,000 a month for it, with a floor of $100 a day per campaign.
